Registering Your School for Service

Follow these steps to register your school for Wolfner Library service:

  1. Call Wolfner Library toll-free at 800-392-2614 to verify that your school is not registered, if you have not already done so.
  2. Complete the Application for Free Library Service: Institutions.
  3. Read the instructions for completing the Annual Student Listing form. Your library service may be delayed if you complete the form incorrectly and/or omit required information.
  4. Complete the Annual Student Listing form.
  5. Read the information about service options for schools.
  6. If you select “on demand” service for one or more formats, contact the Youth Services Librarian and let her know your decision. If you select one of the automatic service options, complete the service options form and mail it with your student listing form.
  7. Mail the completed forms to Wolfner Library, PO Box 387, Jefferson City, MO 65109-0387. DO NOT FAX these forms to the library. The library must have original signatures on file.

The application, annual student listing form, and instructions can be accessed by clicking on the links below. If you cannot access these forms, the library can mail them to you.

Developing a Service Profile for Your School

The final step in registering your school is to develop a service profile that will best meet the needs of your students. An overview of the options is available at the link below. If you choose one of the automatic service options, complete the form at the link below and mail it to the library with your student listing form.

If you have any questions about the forms, or if you need forms mailed to you, contact the Youth Services Librarian toll-free at (800) 392-2614, or by email at wolfner@sos.mo.gov

What Happens When the Library Receives My School's Paperwork?

The library staff is required to check the names of the students on the Annual Student Listing form against the library’s patron database. In accordance with federal regulations, all students using Wolfner Library services in a school setting must have a certified individual application on file with the library. The library staff will notify you which, if any, students are not on file. The Application for Free Library Service: Individuals is available on the web site, or the library can mail one to you for each student.
